Jeane
tte Ferrer, DO has served as Medical Director and Chief of Service
for Supportive and Palliative Care at Houston Methodist Hospital since 201
6. Prior to that she served as Medical Director for Inpatient Hospice unit
and Outpatient teams with Harbor Hospice from 2014-2016. She completed her
degree as Doctor of Osteopathy (DO) at the Western University of Health Sci
ences - COMP in Pomona, CA in 2007. Then she completed a Family Medicine re
sidency at Texas A&M University Health Science Center in 2010 and curre
ntly has been serving as adjunct professor there since 2018. Dr. Ferrer als
o completed a fellowship and Hospice and Palliative Medicine at UT MD Ander
son Cancer Center in 2011. She is board certified in Hospice and Palliative
Medicine and previous was board certified by American Board of Family Medi
cine.
Preeti Mehta, PT, DPT, GCS is the lead ph
ysical therapy clinician at Houston Methodist Hospital for Oncology, Hemato
logy, and Cell/Gene Therapy Units and has been on staff as a physical thera
pist since 2014. She also serves as the Rehabilitation Education Specialist
for Houston Methodist Hospital and is primary faculty in the Acute Care Ph
ysical Therapy Residency Program. She became a Board Certified Clinical Spe
cialist in Geriatrics in 2019. Prior to that, she completed her DPT degree
at A.T. Still University in 2013 and received a Bachelor of Health Science
in Physiotherapy from the Auckland University of Technology in New Zealand
in 2002.
Rajashree Mondkar, PT, MS, CCS is also
a Rehabiliation Education Specialist and physical therapist with Houston M
ethodist Hospital, where she has been since 2016. She has previous experien
ce as Registry Physical Therapist with Harris Health System as well as at S
t. Luke's Episcopal Hospital here in Houston, TX. Rajeshree has also worked
in several positions overseas including Qatar, England, and India. She com
pleted her initial Physical Therapy training for both a Bachelor's and Mast
er's at the University of Bombay in Mumbai, India with an emphasis on Cardi
ovascular and Respiratory Diseases. She became a Board Certified Clinical S
pecialist in Cardiopulmonary Physical Therapy in 1994 and subsequently was
re-certified in 2004 and 2014. She has served as faculty for both the Physi
cal Therapy Critical Care Fellowship and the Physical Therapy Acute Care Re
sidency at Houston Methodist Hospital since 2016. She was also faculty with
the Neurologic Physical Therapy Residency program at Houston Methodist Hos
pital from 2010-2015.
Mitch Maceda, PT, DPT, NCS has 20+ years of experience as a physical therapist in different neurolo
gic settings. He has served as Senior Clinician and Rehabilitation Educatio
n Specialist with Houston Methodist Hospital since 2017. He also is on the
faculty for the Houston Methodist Hospital Acute Care Physical Therapy Resi
dency Program. He completed his Bachelor's in Physical Therapy at Cebu Doct
or's University in the Philippines in 1998 and later received his post-prof
essional Doctorate of Physical Therapy in 2016 from The Evidence in Motion
Institute of Health Professions / Neuro Recovery Training Institute. He is
a Board Certified Clinical Specialist in Neurologic Physical Therapy.
Pamela Glazener, OTR,ATP has been an Occupational T
herapist since 1992. She works for Houston Methodist Hospital and has been
a specialist in treating patients with ALS since 1996, but also works in ac
ute care as part of the multidisciplinary team. She was part of the ALS tea
m at Houston Methodist Hospital that received the Interdisciplinary Team Aw
ard by the Texas Occupational Therapy Association (TOTA) in 2018. She also
received certification as Assistive Technology Professional (ATP) about 9 y
ears ago.
Taylor Dishon M.S., CCC-SLP has been
a Speech-Language Pathologist in Acute Care at Houston Methodist Hospital
since 2018. She represents Speech Pathology amongst a multi-disciplinary Pa
lliative Care team at the hospital which is also inclusive of PT, OT, and t
he Medical Director. She is responsible for supervising clinical fellows an
d graduate student externs and is the team lead in the implementation of th
e International Dysphagia Diet Standardization Initiative in collaboration
with Clinical Nutrition, Food and Nutrition Service, and Rehabilitation Man
agement. Taylor completed her Master's in Speech-Language Pathology with Te
xas Woman's University in 2017 after completing her Bachelor's in Communica
tion Sciences and Disorders at Texas State University in 2015.
